Direct Support Professional

The Direct Support Professional (DSP) in Home and Community-Based Services (HCBS) provides

essential support to individuals with intellectual and developmental disabilities (IDD), physical

disabilities, and elderly clients in home and community-based settings. DSPs assist individuals with

daily living activities, personal care, community integration, and achieving personal goals as outlined in

their individual care plans. This position plays a crucial role in helping individuals live as independently

as possible, with dignity and respect, within their own homes and communities.

Job Responsibilities

  • Required to implement state guidelines and follow Individual Support Plans.

  • Assist individuals with activities of daily living (ADLs) such as bathing,

dressing, grooming, toileting, and eating.

  • Support with mobility assistance, including transfers, walking, or using

mobility devices.

  • Help with instrumental activities of daily living (IADLs), including meal

preparation, light housekeeping, laundry, medication reminders, and grocery shopping.

  • Ensure that individuals health, hygiene, and safety needs are met consistently.

  • Facilitate and encourage participation in community activities, including

    social events, volunteering, recreational outings, and other community-

    based opportunities.

  • Support individuals in developing and maintaining social relationships

    with peers, neighbors, and community members.

  • Assist with transportation, ensuring individuals can access medical

    appointments, social outings, or other community engagements.

  • Assist with medication reminders, monitoring for side effects, and

    ensuring that medications are taken as prescribed.

  • Monitor and report on the physical and mental health status of

    individuals, including recognizing changes in health, behavior, or mood.

  • Maintain accurate and timely documentation of daily activities,

    including progress notes, incidents, behaviors, and other relevant

    observations.

  • Document any changes in the individual’s physical or emotional

    health, as well as any issues related to the delivery of services or

    adherence to the care plan.

  • Complete required reports and submit them to supervisors,

    ensuring compliance with organizational policies and state

    regulations.

  • Ensure that the environment is safe, clean, and free from hazards,

    and assist with light housekeeping duties as needed.

  • Follow safety protocols and emergency procedures in case of

    illness, accidents, or natural disasters.

  • Advocate for the individual’

    s rights and ensure their safety and

    comfort while maintaining a respectful, caring relationship.

  • Promote and advocate for the person-centered philosophy,

    ensuring that individuals' preferences, choices, and goals are

    respected.

  • Act as an advocate for individuals, supporting their autonomy and

    decision-making, and encouraging them to take part in planning

    their care and activities.

    Qualifications

  • High School Diploma or equivalent

  • Additional certifications in first aid, CPR, or

    related fields are preferred.

  • Previous experience in direct support,

    caregiving, or human services is preferred,

    especially in HCBS, long-term care, or

    working with individuals with disabilities.
