Semi-Independent Living

“Intellectual Disabilities Semi-Independent Living Services” means a program which provides services for persons supported with intellectual disabilities, who are able to reside in a housing situation without program staff residing on site. The services of program staff support the home lifestyle of the person supported in order to reside in such housing situations. The program offers and provides regular, and as needed, support services in procuring or monitoring adequate housing for persons supported, supervising home lifestyles, and assisting in development of higher-order independent living skills. The entity licensed is the program delivering the services rather than the individual dwellings for persons supported.

“Developmental Disabilities Semi-Independent Living Services” means a program which provides services for persons supported with developmental disabilities, who are able to reside in a housing situation without program staff residing on site. The services of program staff support the home lifestyle of the person supported in order to reside in such housing situations. The program offers and provides regular, and as needed, support services in procuring or monitoring adequate housing for persons supported, supervising home lifestyles, and assisting in development of higher-order independent living skills. The entity licensed is the program delivering the services rather than the individual dwellings for persons supported.

Services Provided:

1.  Individualized Support Plans:

•       Comprehensive assessment to develop personalized support plans.  Regular reviews and updates to address changing needs and goals.

2.  Residential Support:

•       Assistance with securing and maintaining suitable housing.

•       Support with daily living activities, including meal preparation, housekeeping, and personal care.

3.  Skills Development:

•       Training in life skills such as budgeting, cooking, and personal hygiene.  Opportunities for educational advancement and vocational training.

4.  Health and Wellness:

•       Coordination of healthcare services, including routine medical appointments and medication management.

•       Promotion of physical and mental well-being through fitness and recreational activities.

5.  Community Integration:

•       Facilitation of social and recreational activities to foster community involvement.

•       Transportation services to access community resources, employment, and social events.

6.  Employment Support:

•       Assistance with job search, application processes, and interview preparation.  On-going support to ensure job retention and satisfaction.

7.  Advocacy and Rights:

•       Education on individual rights and self-advocacy.

•       Support in navigating systems and accessing community resources and services.

8.  Emergency Support:

•       24/7 on-call support for emergencies and crises.

•       Development of emergency plans and training on safety procedures.
