ID/DD Supported Living

“Intellectual and Developmental Disabilities Supported Living Services” means a service entity which provides support and assistance to individuals with intellectual and/or developmental disabilities, who live in their own homes or apartments but require staff support and assistance to reside in such housing situations. Service entity staff provides support and assistance on a regular basis in accordance with the needs of the person supported living in the home. No more than three (3) persons supported shall reside in the home. The service entity delivering the support is licensed rather than the homes of the person supported.

4.  Community Integration: Opportunities for social participation and community engagement, including recreational activities, employment support, and volunteer opportunities.

5.  Health and Wellness: Coordination of healthcare services, including medical appointments, therapy sessions, and wellness programs to ensure optimal health and well-being.

6.  Family and Caregiver Support: Resources and support for families and caregivers, including respite care, training, and counseling services to enhance the overall support network.

7.  Transportation Services: Reliable transportation for medical appointments, community activities, and other essential errands to ensure accessibility and mobility.
